Select Plan for Women (Women ages 18 – 44)
Select Plan for Women is a new state-wide medical benefit program.  This program allows women who meet the eligibility requirements to receive free contraceptive services at participating providers. (Program of the Pennsylvania Department of Public Welfare)

Eligibility:

• Women ages 18 – 44
• U.S. Citizen
• Pennsylvania resident
• Have limited or no insurance
• Are not pregnant
• Are not sterilized
• Meet income requirements – 185% of poverty level or below

Services Include:

• Birth control
• Pap tests
• Emergency contraceptives (“Morning After Pill”)
• STI testing & treatment
• HIV testing & counseling
• Breast exams
• Routine Women’s Health Care

An application process must be completed to enroll in Select Plan for Women.  Lebanon Family Health Services is happy to enroll eligible women into this program.  To accurately complete the application at our office, please bring the following items (if unable to locate, please bring the information that is readily available to you):

• Social Security Number
• PA Driver’s License or state-issued identification card
• Birth certificate or U.S. passport
• A recent pay stub or employer’s letter stating pay for a typical pay period for all working family members in your household

*Household size includes the woman’s legal spouse and/or dependent children*

• Proof of other income, such as child support
• Insurance card (if you have health insurance)
